We are seeking a Team Leader who has experience of working with LD and complex challenging behaviours. The ideal candidate will have experience of working with Domiciliary and Supported Living care packages and have experience of working with Carass or other erostering system. Overall Purpose of Job

To work as part of a dedicated team to provide a high quality support service to people in their own homes in accordance with their agreed plans.

MAIN DUTIES & RESPONSIBILITIES

  • To deputise for the Care Manager as required

Carry out the day-to-day responsibilities of managing the agency in the absence of the manager

  • Undertake assessments of people’s care needs

Receive initial service enquiries and assessment details.
Carry out a detailed community-based assessment of potential client’s needs. Produce comprehensive care/support plans with client, relevant professionals and relatives

  • Carry out individual and environment risk assessments

Undertaken environmental and risk assessments and include all relevant information to minimise risks in client’s care plan

  • Produce staff work schedules for allocated areas

Produce planned work schedules for allocated area/team and ensure care and support is provided, as and when required, for all clients

  • Monitor service user care/ support plans and all data and files

Carry out regular monitoring of records and files to ensure all relevant information is maintained to a high standard and issues are addressed promptly

  • Actively participate in care reviews and planning meetings
  • Supervise and motivate a team of support staff
  • Carry out staff practice observations (announced & unannounced)
  • Identify staff training needs and participate in training sessions
  • Ensure staff operate within the current regulations, codes of conduct and good practice guidance
  • Participate in recruitment and selection process for support workers and in training sessions
  • Carry out On-call duties on a rota basis

ASSOCIATED TASKS & RESPONSIBILITIES

  • Deliver direct care to people in their homes as and when required
  • Participate in working/projects groups as required 
  • Attend relevant training courses to develop and maintain client and organisational based skills
  • To carry out other duties as from time to time may be deemed appropriate to this post.
